The vendor is required to provide for an operational medical director (OMD) for the city’s fire department.
- The OMD will have the ultimate authority over patient care delivered by the emergency medical services (EMS) providers certified to practice under the OMD license.
- The fire chief and the OMD agree that all department providers will be trained and required to follow specific medical protocols as adopted or implemented by the OMD for department.
- The OMD shall provide oversight and authority for the providers to practice at their certified level as a member of the department after all agency and OMD training requirements are met
- Department agrees to participate in a performance improvement (pi) process as directed and defined by the OMD.
- The OMD shall be informed of patient care errors and/or omissions by providers as deemed necessary.
- The OMD shall keep his/her current license to practice medicine in the state of EMS credentials and ancillary EMS certifications on file with department.
- The OMD shall conduct potential new hire candidate testing as needed
- The OMD shall provide medical oversight and expertise regarding appropriate training, continuing education, and performance improvement by reviewing and analyzing EMS effectiveness, system trends, and needs in an effort to ensure EMS system excellence
- The OMD may suspend certified EMS personnel from medical care duties pending review and evaluation.
- The OMD shall act as the physician on record for ordering medical equipment and supplies.
- The OMD shall maintain appropriate professional relationships with the local community including but not limited to medical care facilities, emergency departments, emergency physicians, allied health personnel, public health, law enforcement, fire protection and dispatch agencies
- The OMD shall follow all local, state, and federal laws and regulations.
- The OMD shall provide proof of completion of an EMS medical direction program approved by the office of EMS with in the past five years.
- Medical oversight:
• The OMD shall spend a minimum of 8 hours per quarter, conducting “ride-a-longs” on the medic units, and EMS supervisor vehicles in order to facilitate an effective working relationship with field providers, provide on-scene medical direction (as needed), assess compliance to protocols and policy, observe and evaluate the quality of patient care, and be a mentor to department personnel
- System evaluation:
• Collaboration and consultation with department chief medical officer as deemed necessary to maintain an ongoing performance improvement (pi) program in accordance with § 12 vac 5-31-600.
• The pi program shall be integrated and include activities related to patient care, communications, and all aspects of transport operations.
- Provider education:
• The OMD shall maintain close working relationship with the EMS education coordinator, providing guidance to training personnel, incumbent providers, and recruits.
• The OMD shall establish educational objectives and review and approve requirements for initial training and continuing medical education (CME) for department personnel.
• The OMD shall integrate performance improvement outcomes and initiatives into the ongoing training and continuing education of department personnel.
• The OMD shall provide promotion of opportunities for additional education of department personnel by establishing and maintaining collaborative relationships with academic institutions and the medical community.
• The OMD shall conduct quarterly (four hours per quarter) OMD continuing education training.
• The OMD shall conduct annual ALS training (24 hours per year) to incumbent providers. - The OMD shall attend meetings, as scheduled, to include:
• Monthly department EMS performance improvement meeting (minimum of two hours/month)
• Monthly department field medical officer meeting (minimum of 2 hours/quarter)
• Monthly department senior staff meeting (minimum of two hours/month)
• Monthly department OMD and CMO meeting (minimum of four hours/month)
• Quarterly regional medical direction committee meeting (minimum of three hours/month)
• Quarterly emergency medical dispatch steering committee meetings (minimum of two hours, four times/years)
- The OMD shall provide medical oversight, expertise and direction to department special medical operations teams, to include, but not limited to:
• Tactical paramedic program (swat)
• Dive medicine program
• Hazardous materials paramedic program
• Mass care and shelter medical program
• Mass casualty incident management program
• Special event medical team program
• EMS bike team - Performance improvement:
• The OMD shall provide medical oversight, expertise and direction to coordinate activities for a proactive, productive and consistent EMS performance improvement program.
• The OMD, in coordination with the department EMS performance improvement officer and the department chief medical officer, through reviewing and analyzing EMS system effectiveness, system trends and community need shall develop EMS system performance benchmarks.
• The OMD shall conduct timely and on-going clinical practice and ePCR reviews of department personnel to ensure the delivery of quality pre-hospital care.
• The OMD shall conduct timely and on-going emergency medical dispatch reviews to ensure quality emergency medic al dispatch services.
