- Provider network
• Provides the network(s) that agency associates will be accessing by their employee address.

• Provide average discount percentages by service, for each network
- Claims processing and administration
• Provide an organizational chart of your claims department
• The administrative service targets and actual performance in each of the areas listed below:
1. The quality management program of the claims administration function
2. The organization, methods and procedures for responding to routine claim inquiries from associates
3. Provide your procedures for claim appeals, which must comply with PPACA
4. Agency will divert all appeal rulings to the carrier and will not be making any overrides or appeal decisions.
- Reporting and website capabilities
• All dental utilization reports and formats available to agency.
• Please provide samples of:
1. Monthly reporting package
2. Quarterly reporting package
3. Annual reporting package
- Funding
• Please outline your procedures for claim adjudication and the client’s responsibility for funding claims and how often.
- Communication
• Providing transition and marketing and educational communication materials
• Agency will require a communication effort to announce the administrative services provided by its selected vendor and to educate participants about how to access the selected vendor’s services.
1. Please explain what, if any, transition communications are included in the services and fees your organization is proposing for agency
2. If transition communications are not within the scope of your proposed services and fees, simply indicate that these communications are out of scope.
3. If you do not offer a “standard” transition campaign, please discuss the typical components of a transition campaign and also describe how your services are priced

• Provide level of experience and at least three references for engagements with clients including experience with clients in other public sector and/or k-12 environments.
• Include any urban school districts that you have engaged with your product.
• Please include contact information for all references.
• Provide references of clients you have lost in the past two years
• Include reason for loss of client, scope of services provided, contact name, title, telephone number and e-mail address.
• Please be sure to communicate to your references that agency will be contacting them.
