Evidence-Informed Strategy Technical Assistance Support Services

The vendor required to provide evidence-informed strategy technical assistance support services to improve hypertension and diabetes prevention, management, and control, and reduce disease risk factors through the implementation of one or more evidence-informed strategies, including but not limited to community-clinical linkages, value-based payment models, team-based care approaches, and advancing the use of health information technology and data monitoring.
- Cardiovascular disease (CVD) and diabetes are chronic diseases that affect the quality of life for many Coloradans.
- There are key modifiable risk factors for other chronic conditions including Alzheimer’s disease and related dementias. 
- Risk reduction of CVD and improved diabetes and hypertension prevention and management are best addressed by management of disease in combination with the mitigation of social support barriers through coordinated care by a diverse team of health professionals.
- Coloradans will benefit from this project by receiving high quality and timely chronic disease prevention, management, and control services, and reduced barriers to community-based social support services.
