Chronic Disease Self-Management Programming Services

The vendor required to provide chronic disease self-management (CDSM) programming in rural and state by training additional lay leaders, offering community workshops, and marketing for local participation.
- Requirement:
•    Maintain a license for an evidence-based chronic disease self-management (CDSM) program throughout the project period. 
•    Offer CDSM lay leader trainings. 
•    Plan for CDSM workshops in rural state communities and offer technical assistance. 
•    These may be held in-person or virtually. 
•    Develop and implement a marketing plan to engage local participation in CDSM workshops, especially in rural communities.
