Benefits Life, Disability and Family Medical Leave Administration Services

The Vendor is required to provide employee benefit plans including group life, accidental death and dismemberment (AD&D), administrative services only short term disability and long term disability insurance, and family medical leave administration.

• Proposal for group long term disability
• Proposal for all ASO short term disability administration
• Proposal for family medical leave administration
• Signed proposal compliance letter
• Signed plan design confirmation
• Completed and signed questionnaire(s)
• Proposed implementation timeline for the city.
• Copy of your policy assuring member satisfaction
• Samples of all standard and optional reports you are proposing to provide on an account specific basis
• Copy of your banking services agreement
• Copy of your customer satisfaction survey
• Copy of your administrative services agreement or insurance contract that will be in effect January 1, 2027
- Be comprised of individuals with specialized knowledge of the proposing company’s:
• Claims and eligibility systems
• Systems reporting capabilities
• Claims adjudication policies and procedures
• Administrative services contract wording
• Standard and non-standard banking arrangements
• Relationships with third parties.
