Technical Assistance, Training, and Implementation Support Services

The Vendor is required to provide technical assistance, training, and implementation support services for include:
• Programs will be required to maintain accurate reporting and case records according to regulation and program guidance.
• A unique consolidated fiscal report (CFR) program reporting code will be created for tracking of these funds.
• A unique CFR program reporting code has been created for tracking of these funds; funding will be reported under code 1760.
• Expected to ensure continuous quality improvement of services, including regular monitoring and evaluation of outcomes.
• To support these efforts, it is expected that providers have a quality, supervisory, operational and it and data infrastructure to routinely self-monitor and ensure ongoing quality improvement of services, including analyzing utilization review findings and recommendations.
- Implementation technical assistance
• Staffing the collaborative care model (COCM) team – providing practices with sample language for job descriptions for each member of the clinic implementation team.
• This includes behavioral health care manager (BHCM), psychiatric consultant, PCP champion, billing and data leads.
• Support in the hiring process as applicable.
• Support sites in identifying a population health registry for tracking and managing their COCM patients as the model requires, including:
o Tracks clinical outcomes and progress at the patient and caseload levels
o Supports treatment by summarizing patient progress in an actionable way.
o Enables efficient psychiatric consultation, allowing BHCMS to easily prioritize patients for systematic caseload review.
o Supplies reports to program managers and clinical leadership to monitor progress toward goals, including processes of care, quality of case and patient outcomes metrics.
o Supplies de-identified reports to outside auditors to demonstrate regulatory compliance, intensity of service, staffing ratios, process measures
• Development and implementation of all relevant workflows, including universal screening and other methods of patient identification, COCM workflow development tailored to the needs of the practice, and suicide safer care pathway development.
