The Vendor is required to provide information regarding available housing, reentry, and supportive services for homeless, formally justice impacted individuals.

• Available housing options include emergency shelter, transitional housing, sober living, and permanent supportive housing.
• Geographic areas serviced and current service capacity.
• Housing stabilization continuum.
• Eligibility requirements and intake processes.
• Reentry and supportive services that are offered, including case management, behavioral health services, substance use services, employment assistance, life skill development, and identification document support
• Details of any other support services offered.
• Supervision levels or program restrictions, if applicable.
• Existing partnerships with correctional agencies, governmental entities, or community organizations.
• Ensuring individuals are connected to appropriate care based on assessed needs.
• Benefits acquisition and healthcare navigation.
• Recommended best practices or considerations related to serving homeless, justice impacted individuals
• Access to meals.
• Any additional information department should consider regarding available services.
• ‘Best’ way for the state to collect and compare costing information from suppliers, to allow the state to compare proposals ‘apples-to-apples’.
• Any other factors, issues, or items the state should be considering at this time.
