Medical Supplies Services

The vendor required to provide medical supplies on an as and when required basis.
- Supplier’s responsibilities
•    Notify city within 24 hours of receipt of a manufacturing recall. 
•    That delivery times required of this contract are met. 
•    Supplying the drug identification number (“DIN”), manufacturer certifications, testing reports, and all other product documentation requested by city within 24 hours of the request. 
•    The supplier must have the ability to supply all items listed in the schedule of prices in the bidding system. 
•    That the supplier must be an approved distributor of the manufacturer of the items marked as “no substitution” in the pricing forms in the schedule of prices. 
•    These manufacturers are:
o    Braun (line items 10 and 11 in pricing forms 1 to 5)
o    Contour (line items 16, 17 and 18 in pricing forms 1 to 5)
o    Accu-Chek (line items 19 in pricing forms 1 to 5)
o    QUIK clot (line items 32 in pricing forms 1 to 5)
o    3m (line items 41 and 42 in pricing forms 1 to 5)
o    Laerdal (line items 39 in pricing forms 1 to 5)
o    O-two medical technologies (line items 53 in pricing forms 1 to 5)
o    MMSI (line items 56 in pricing forms 1 to 5)
o    Sam medical (line items 58, 61, 72, and 73 in pricing forms 1 to 5).
