The vendor is required to provide pharmacy will make available to the facilities on loan (BLH – qty. 6, GWH – qty. 4), at no charge to the facilities, equipment and reference material necessary or useful to provide proper pharmaceutical services.
• Qty. ten (10) medication carts with computer and tablets and lockable narcotic bins;
• Qty. ten (10) treatment carts including computers and tablets;
• Qty. ten (10) silent knight pill crushers (one per unit);
• One hard copy for each home of the pharmacy policy and procedure manual as well as electronic version;
• Standard forms (e.g., ADT forms, mar sheets, reorder sheets, physician order sheets, etc.), any required binders;
• Qty. ten (10) fax machines (one for each unit).
• Any costs associated to maintain the faxes to be covered by pharmacy.
• Qty. ten (10) Digi-pen available or similar technology for order processing (one for each unit);
• The pharmacy must integrate with the county owned e-boxes.
• The pharmacy must be able to provide a pharmacist to lead the collaborative medication reconciliation process for admissions and readmissions;
• Any other equipment and maintenance of equipment required for eMARS, including but not limited to ten (one per cart) computers and all hardware required for attachments to carts and future replacement of same.
