The Vendor is required to provide pharmacy solution shall also provide consulting services, through, but not limited to, participation in various committees at each home and reviews of the homes’ pharmacy policies and procedures.
- Pharmacy Service Staff Team
• Pharmacist and pharmacy technician working on behalf of the successful proponent must be registered with the college.
• They must have specialized knowledge and skills to bring a high level of expertise to the pharmaceutical care of the residents residing in long-term care homes and must have expertise in the management of complex clinical conditions to achieve the optimal therapeutic outcome for the resident.
- Pharmacy Staff
• The pharmacy service staff team will consist of a dedicated pharmacist for the home, pharmacist consultants, pharmacy technicians and pharmacy administrative staff or dedicated account manager and fulling the requirements of the act.
• The pharmacy consultant will be available at least four (4) scheduled days per month in the home to do rounds and be available for consultation with the prescriber and/or the DRC and/or designate.
- Medication assessments and re-assessments at time of admission, quarterly, annually and when indicated by the physician/nurse practitioner and care team, in conjunction with the physician and the nurse.
- Attendance at resident interdisciplinary conferences as required and provide necessary resident and family consultation. Education to residents and families on medications (including benefits and risks) and the provision of printed material (as needed).
- Pharmacist-led medication reconciliation at time of admission, support with medication reconciliation on re-admission, transfer and discharge.
- Support with monitoring drug to drug interaction.
- All pharmacists will be involved in emergency planning, palliative care plan, pandemic and fall preparedness meeting and other resident care services initiative.
- Home and Divisional Support/Resource
• The pharmacist consultant will act as a resource to prescribers in relation to drug DE prescribing and drug utilization or any quality improvement programs.
• Support region during compliance with the ministry of long-term care, accreditation country survey and other regulatory bodies.
• Support home in the annual medication management program evaluation.
• Participating in the disposal of discontinued medication – narcotic/controlled and nonnarcotic/non-controlled medications
- Assessments may include on-site or remote examinations of Proponent's internal controls (such as business, security and information technology practices) relevant to the systems and applications.
