The Vendor is required to provide pharmacy consulting services to the residents and staff of all eight agency.
- Provide orientation to home clinical and administrative staff regarding pharmacy consulting services proposed by the responder;
- Evaluate the operational, clinical, and regulatory needs unique to each Home; and
- Develop a site-specific pharmacy consulting action plan.
- Home-specific quarterly reports summarizing pharmaceutical services, findings, trends, and recommendations; and
- Staffing resources will be allocated to meet service delivery requirements across all participating Homes, including:
• Scheduled site visit coverage;
• Reporting requirements; and
• Availability for consultation support.
- The ability to perform additional pharmacy-related projects upon request, which may include:
• Medication pass observations
• Medication room audits
• Targeted drug regimen reviews
• Medication safety or quality improvement initiatives.
