The Vendor is required to provide prompt delivery of tele pharmacy services including drug distribution and clinical pharmacist services primarily during after business hours (evenings, overnight, weekends, and holidays), and also to cover pharmacist absences during regular business hours.
- Requirement:
• Clinically review and enter medication orders for patients at QGH and KHC into the health information system.
• Identify, address, and resolve drug therapy problems by consulting with healthcare professionals.
• Provide clinical and drug information support to state healthcare staff as part of drug distribution duties.
• Perform and document medication reconciliation for admitted and discharged patients at QGH and KHC, including the identification and resolution of discrepancies.
• Respond to and triage after-hours calls and collaborate with on-call pharmacy technicians as needed for medication dispensing.
• Provide patient care handover and transfer of accountability to the clinical pharmacist.
• Assess patient medical charts and offer clinical pharmacy recommendations to physicians.
• Provide patient/family/caregiver education regarding medication therapy.
• Assist with discharge planning and patient transitions to the community.
• Attend team meetings as required.
