The Vendor is required to provide for pharmacy services to home including any services provided under the guidelines of the special care home guidelines within authority.

• Describe participation in quality improvement initiatives
• Describe participation in medication incident reviews and reporting including agency (Community Pharmacy Professionals Advancing Safety in state)
• Identify and minimize diversion opportunities within medication process
• Demonstrate evidence of ability to provide medications in a safe and efficient manner
- Clinical Collaboration
• Describe on-site medication management process for each resident’s medications
• Provide consultation for the development of pharmacy related policies and procedures
• Participate as a member of the interdisciplinary facility team during path of life meetings/care conferences and medication reviews
• Describe active communication methods with the healthcare team to ensure safety and health of the residents and staff
- Billing procedures that would be used – frequency, responsibility to collect, credit process for returned medications that can be re-dispensed, sample bill, costs for repackaging, any additional charges
- Monitor dosage system to be used, also include dosage system for PRN and multi-dose medications
- Hours when pharmacy services would regularly be available and method of obtaining after-hour Emergency Services
- Audit process for medication room, medication system, controlled substances, night cupboard, and medication reconciliation.
- Process for night cupboard/Stat Box process including but not limited to naloxone, glucagon, and epinephrine.
