The Vendor is required to provide pharmacy consultant services for include:
• Review and evaluate pharmaceutical services, by assisting the agency identify, evaluate, and address medication issues that may affect veterans/residents care, medical care, and quality of life.
• Collaborate with the agency and the medical directors to develop, implement, evaluate and revise (as necessary) the procedures for the provision of all aspects of pharmaceutical services.
• Assist as needed with the coordination of pharmaceutical services if and where multiple services providers are used.
• Assist as needed to develop intravenous therapy procedures.
• Assist as needed to develop intravenous therapy procedures.
• Identify any drug therapy irregularities such as:
• Use of medication without supporting diagnosis
• Use of inappropriate medication dose
• Concurrent use of medication that could potentially interact
• Duplicate therapy
• Continual use of medication that is no longer required
• Presence of medication-related adverse reaction
• Use of medication that is not cost-effective
• As defined by the medical director
• Monitor the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data and other pertinent veterans/members information.
• Communicate results of therapeutic drug monitoring activities to appropriate personnel (i.e. Physicians, nurses and administrators.)
• If expected provider response is not received, inform the director of nursing or designee.
