The vendor is required to provide from established medicare shared savings program (MSSP) accountable care organizations (ACOs) to form a partnership that will support our entry into the MSSP for the 2026 performance year.
- The selected ACO must align with our educational and clinical mission, deliver measurable population health outcomes, and help our organization achieve financial and operational goals under an upside-only MSSP arrangement.
- We are seeking transparent, experienced partners that demonstrate a strong track record of managing MSSP performance and integrating academic and primary care-based organizations into their networks
- Key responsibilities include:
1. Onboarding and MSSP participation preparation
• Guide the practice through the MSSP enrollment process for 2026
• Facilitate the development of required reporting and contracting documentation
• Assess and recommend tin segmentation or alignment strategies to optimize attribution and cost performance
2. Data integration and analytics
• Provide access to a secure data platform for population health reporting, quality tracking, and claims analytics
• Deliver regular, actionable performance reports to practice leadership and providers
• Support integration with EHR systems and workflows where possible
3. Care coordination and management
• Offer tools, resources, and guidance for risk stratification, patient outreach, and care gap closure
• Collaborate with practice leadership to support care coordination personnel, either through direct staffing or shared resources
• Align care models to reduce unnecessary utilization and improve chronic care outcomes
4. Provider and team education
• Deliver ongoing education and training for physicians, apps, and clinical staff on value-based care principles, ACO operations, and MSSP quality reporting
• Support residency training needs, including alignment with teaching workflows and documentation standards
• Facilitate participation in ACO quality improvement collaboratives, webinars, or other engagement opportunities
5. Compliance and regulatory support
• Ensure full compliance with CMS MSSP requirements and reporting standards
• Support the practice in understanding and fulfilling quality reporting and patient attribution requirements
• Assist in adhering to applicable university, HIPAA, and medicare regulations
6. Financial alignment and transparency
• Provide a clearly defined shared savings model that includes distribution methodology and provider-level incentives
• Clearly disclose all administrative or participation fees
• Offer recommendations and support for population health investments that can enhance MSSP performance
7. Ongoing collaboration and governance
• Maintain regular engagement with practice leadership through joint governance meetings, steering committees, or advisory groups
• Offer strategic planning input and performance improvement guidance based on data and outcomes
• Support continuous feedback loops to improve partnership effectiveness.
