The Vendor is required to provide group critical illness insurance coverage is elected voluntarily by district employees with no employer contribution.
- The group critical illness insurance plan shall include:
• No waiting period
• Waiver of premium
• Portability
• Rate Guarantee
• Guarantee issue
• Age rated
• No pre-existing conditions limitations
• Minimum $5,000 up to $50,000 benefit for Critical Illnesses
• Reoccurrence benefit
• Wellness benefit
• No reduction in benefits due to age
• Lump sum benefits paid to the insured.
