The vendor is required to provide that medical administrative activities consultation and training services for to pick-up at district facilities, all specified devices, perform the required activities as to each such devices, and to dispose of such devices by resale or by recycling.
- White glove pick up:
• The pickup of devices from designated location(s) and packaging devices for shipment
• The supplier will inspect, evaluate, and take inventory of the devices
• During normal school days/hours, and remittance needs to be made prior to pick-up of devices
accept and promptly pick-up all designated devices. selective acceptance and pick-up will not be allowed.
• The devices that are picked-up will be accepted by the supplier “as-is” and “where-is”
• Supplier employees involved in the picking up of specified district mobile devices must be full time employees of the supplier and must wear a name badge

- Supplier responsibilities during and after pick-up:
• to each such district mobile device picked up, supplier will perform the following services
• These services must all be performed in the continental united states:
1. The hard drive at least to the passes of zeroing out the hard drive, such that any data stored on the hard drive cannot be reconstructed; log the date and time of completion of the wiping process for each device; and provide written certification of the process used for each device being wiped and that each device resold or recycled has undergone this process.
2. Accomplished on devices both being resold and being recycled. hard drive and processors in devices being recycled should be removed, wiped and shredded.
3. Remove all identifiable etchings or tags referencing the district prior to the units either being resold or recycled.
4. Individually package each device in boxes provided by supplier.
5. Package and box items in master cartons provided by supplier.
6. Palletize and secure products for shipping.
7. Load devices into transportation vehicles – either company vans, trucks or third-party carrier vehicles
- Unusable devices:
• A device picked-up by the supplier from the district is unusable, supplier is not allowed to re-sell the device and all such unusable devices must be recycled in an environmentally responsible manner.
• Devices recycled must be reported to the district, including information set forth in section below. no devices specified for pick-up shall be returned to the district.
• Recycling must be completed in the continental united states.
- Grading scale
• Include in your proposal upload documents showing your grading scale to determine the final value to be paid to the district for each of the devices.
• The grading scale shall set forth the values for each type of mobile device, along with the corresponding elements/conditions that qualify a device for a given value.
- Itemized report
• Supplier will provide to district within 30 days of pickup, an itemized report after grading each device, report shall include at least the following information:
o Device type
o Specific model
o Serial number
o Final grade and value
- Point of contact
• Provide the district with a point of contact for the duration of the contract to work with assigned district staff.
• The authority to act for the supplier in the administration of the contract
• Provide to supplier a list of those district staff members are authorized to designate devices to be removed by supplier.
