The vendor is required to provide providing on-site optometry services by a licensed optometrist who has experience in treating persons with intellectual and or developmental disabilities, mental illness, substance use disorders and neuro-medical needs.
- Provide on-site optometry services to individuals residing at Broughton Hospital who need these services included:
• Provide optometry services for inpatients onsite at brought on hospital monthly (up to bi-monthly).
1.Perform eye exams and vision tests
2.Prescribe and fit eyeglasses (RX and OTC)
3.Monitor eye health related to chronic medical conditions
4.Manage and treat chronic eye conditions
• Clinics shall be full day clinics (8-hours).
- Provide high quality optometry services to approximately 300+ patients of the BH.
- 100% of identified/referred residents will receive optometry services during the clinics held per month.
- 95% or above satisfactory rating will be received by feedback and comments from the referring medical provider.
- Monitor security measures, as outlined in federal rules and state security standards and report reaches: in the event of a breach;
• Take prompt corrective action to mitigate any risks or damages; and
• Take any action pertaining to such unauthorized disclosure required by applicable federal and state laws and regulations.
