The vendor is required to provide public health services and foster collaborative actions that empower our community to improve its health and safety.
- The health and wellness of individuals and families through innovative practices.
• Increase capacity and support for agency providing community-based health access services
• Improve access to health care and support services
• Promote health education and empowerment
- The two primary areas of focus include:
1. Access to care/support services
• Agency will provide navigation health services.
• Examples include primary care, health screenings, immunizations, maternal and infant health care, and services that address ongoing barriers to health care
o Care navigation - assisting individuals in finding appropriate health care providers, scheduling appointments, understanding treatment options, navigating health care systems, completing paperwork, and understanding medical options
o Care coordination – connecting individuals with community resources such as health care services, social services, and transportation assistance.
o Support and advocacy – providing ongoing support to individuals in treatment, advocating for their needs, and addressing barriers to care.
o Outreach and engagement – identifying individuals in need of health care services, educating them on available services, and encouraging preventative care.
o Cultural competency – providing culturally sensitive health information to support based on community language, culture, and social norms
2. Health education
• Agency will provide evidence-based disease prevention and management education.
• Examples include chronic disease and infectious disease education, and preventative mental, social and physical wellness education
o Health education and promotion – conduct workshops, seminars, and awareness campaigns. include nutrition, physical activity, and preventive screenings
o Communicable and non-communicable disease prevention programs – provide vaccination clinics, screenings, infectious disease education, chronic disease management programs (obesity, diabetes, asthma, hypertension, cardiovascular disease) and preventive mental, social, and physical wellness education
o Outreach and engagement – identify underrepresented individuals in need of health education services.
o Partner with community organizations to promote health awareness and implement preventive programs.
