RFP Description

The vendor is required to provide workers’ compensation private investigators to assign work to vendors in the pool based on a consideration of the geographical location of the matter to be investigated, workloads of the vendors, particular expertise of individual vendors, and evaluated performances of vendors on past and ongoing assignments.
- Surveillance Media (CD/DVD)
• All surveillance media must be provided in CD/DVD format and of good, clear quality, in focus, and free of any sounds or noises.  
• All media must include the date and time imprinted/displayed on the media during any filming activity.
• All locations, times, dates, and activities of the individual or business under surveillance must be documented in the body of the investigative report.  
• All individuals that are the focus of surveillance shall be identified.
• This information must also be referenced in the daily investigative log.
• All surveillance media submitted to agency must be properly labeled.  
• All media labels must include: the first and last name of the individual, or complete name of the business, being videotaped; workforce safety and insurance’s claim number or employer number if applicable; the dates and times of surveillance that are on the media CD/DVD; and, the total time of the CD/DVD that was used.
- Still Photographs
• Photographs submitted which contain the subject or circumstances surrounding the investigation must be clear and relevant to the investigation being conducted.
• The investigative report must include all relevant information referring to all of the photographs submitted to agency.
• All investigative reports submitted to agency must be received along with the corresponding evidence and investigative billing.
• All investigative reports must be clear, concise and contain all the events, dates, times, locations, and information discovered during the investigation;
• All investigative billing must include a page summarizing the totals for each investigative activity billed.  This must include a line item for each investigative activity listing the total amount charged and how this total was reached;
• All investigative billings must document in chronological order the dates of activity, start and end times for each date, and a brief explanation of the investigative activity, i.e. surveillance time, travel time, etc.
