Health Risk Screening Tool Screening Services

The Vendor is required to provide the health risk screening tool (HRST) as the desired method to be used to detect health risks and destabilization early in individuals with intellectual and developmental disabilities (I/DD) and therefore assist those responsible for support and planning the needs of these individuals.
- Responsible for providing intellect ability trained raters to perform HRST screenings.
- Initial heath risk screen tool (HRST) screening performed by intellect ability trained rater in fy26 (September 2025 – June 2026) for department residents – cost per person; estimated 650 individuals
- Annual HRST online application license (September 2025 – June 2026) for department residents that received HRST screening – cost per person; estimated 650 individuals 
- Annual / secondary heath risk screen tool (HRST) screening performed by intellect ability trained rater in fy27 (JULY 2026 – June 2027) for department residents – cost per person; estimated 650 individuals
- Annual HRST online application license (July 2026 – June 2027) for department residents that received HRST screening – cost per person; estimated 650 individuals
- Implementation process (i.e. Information webinars, access to instructional videos for department staff) - total cost.
