The Vendor is required to provide or the supply, delivery, implementation, support of a commercially available off-the-shelf form builder software solution.

•    Ability to seamlessly integrate with Salesforce
•    Manage complex form logic
•    Ability to combine multiple forms
•    Comply with state privacy and data storage requirements
