The Vendor is required to provide a wellness portal solely for the use of City employees and retirees (covered under our medical plan), as well as their covered dependents.
- This portal requires both the employee and spouse or qualified domestic partner to set up a username and password. The portal is also available through mobile devices.
- This is important as we have large populations of employees who do not have computer access. Once in the portal they can not only complete their Personal Health Assessment, but also manually selfreport activity, while competing in a quarterly challenge.
- Participants also have the option to link a device or app in order to track steps or minutes of activity. In the portal, participants can also earn additional dollars through completion of quarterly education (taking a course and completing the quiz.
- The participant must score 80% or higher on the quiz in order to receive credit.
- Additional options in the portal include daily mini challenges, setting SMART goals, reporting preventive care visits, and reporting volunteer time in the community among other activities.
- Currently the Benefits Team has the ability to review and approve documents that are uploaded by participants for some of the incentives.
- The portal vendor does receive weekly files containing the biometric results performed at the clinic—not one single file.
- The results of those metrics determine the incentive each participant receives quarterly for the upcoming year.
- Wellness Portal services to be provided by the contractor:
• Fully customizable site with employer having the ability to make updates and changes and describing, in detail, what can be customized.
• The ability to upload educational materials like videos, webinars, articles, etc. as well as content already available for use on the site.
• The site needs to have the ability to track physical activity (steps and minutes), program participation, and biometric results, while keeping members engaged with periodic behavior change campaigns and challenges.
• Integrate with various fitness devices (smartwatches, fitness bands, and smart rings to upload step count and/or minutes of activity
• Accessible by the employee, employer, and the disease management educators who are employed by our onsite clinic provider.
• Accessible on a mobile device (please describe whether accessibility is responsive design or through an application)
• Coordinate administration of our health incentives program with onsite clinic vendor
• Coordinate reporting with onsite clinic vendor, and ability to house the biometric results and wellness scores.
