Health and Risk Screening Tool

The Vendor is required to provide to gain familiarity with currently-marketed products and services for a web-based screening tool to identify and assess health and welfare risks, and support needs across all major life domains.
- This includes evaluating health and well-being, safety and behavior, activities of daily living, decision-making capacity, and mental and emotional health.
- A secure web-based system that identifies risks before a crisis occurs
- The Tool will be used by non-clinical staff
• Primary users for screening are case managers
• Results utilized by the participant’s team, agency administrative teams, and clinical supports
- Guide Appropriate service planning and resource management
- Support Current Systems.
