• For ad hoc single document request for agency, the transcribed document will be sent and emailed to agency.
- Monthly summary report
• Provide a monthly breakdown report to health equity operations by the 10th workday of each month following the previous month in which services were provided, detailing a breakdown of all services rendered.
• The report shall include, but is not limited to:
1. Name of document
2. Input pages
3. Output pages
4. Cost of taglines (if any)
5. Postage
6. Shipping and handling
7. Tracking total amount of transcription services.
