Medical, Rx, Dental, HSA and Vision Administrative Services Only

The Vendor is required to provide Administrative Services Only (ASO) for Medical, RX, Dental, and FSA & Vision.
- The current medical, pharmacy, dental and vision plans
- Services include:
•    Demonstrate adequate financial resources or the ability to obtain such resources by providing the most recent fiscal year’s statement.
•    Demonstrate an ability to comply with required or proposed delivery schedule.
•    Demonstrate a satisfactory record of performance by providing at least five (6) references where like products and/or services have been supplied by the firm; preferably municipalities of the same size and demographics. Please include name of firm, address, telephone number and name of representative.
•    Demonstrate a satisfactory record of integrity and ethics in the profession by providing documentation supporting at least 5 years in the field.
•    Be otherwise qualified and eligible to receive an award by providing appropriate company licenses, certifications, key staff assigned to work with the Town and other pertinent resources.
- Financial Statement – most recent published financial statement and/or best insurance rating, including financial size category.
- Samples – Enrollment materials, provider directories, ASO Agreement, ID Card, Explanation of Benefits (EOB), reporting package.
