The vendor is required to provide social supports to pregnant and postpartum individuals at the highest risk for poor birth outcomes.
- Requirement:
• Develop and coordinate a program to enhance the social support for pregnant and postpartum individuals at the highest risk for poor birth outcomes.
• The proposed program can enhance an existing program offering.
o Social supports can include but are not limited to prenatal and postpartum case management, maternal health education, maternal mental health support, family programming, implementation of support groups, childbirth classes, parenting classes, nutrition support, and physical health and wellness support.
o Provide prenatal and postpartum home visiting services which can include support for breastfeeding, education about postpartum warning signs, education about prenatal hypertension, and education about gestational diabetes.
o Education about self-management can be discussed (e.g., blood pressure monitoring, glucose monitoring) but no additional medical management can occur
o Program incentives or promotional items with prior approval.
o Examples include but are not limited to prenatal and postpartum journals, cookbooks, diapers, lactation supplies, snacks for programming, manual breast pumps (excludes the purchase of electric breast pumps), etc.
• Provide a description of how program activities will be promoted and implemented.
• Include program recruitment and retention strategies, and an explanation of the expected benefits of the proposed programing.
• Identify key proposed personnel and their role with the program.
• Demonstrate experience with promotion strategies and message distribution.
• Provide a timeline for program promotion, recruitment, and implementation from the start of the Agreement period through the end of the first contract year.
• Provide a description of any real or perceived risks associated with the proposed program.
• Identify those activities which can be undertaken to reduce, mitigate, or eliminate the risk, and identify the associated responsibilities.
• Provide a detailed evaluation plan that includes qualitative and quantitative metrics
• For all staff assigned to work on the project, the contractor shall conduct a background check or investigation consisting of a national criminal database search covering misdemeanors and felonies.
• Release information from the search to the County, in the event that it is required.
