The vendor required to provide contracted, community-based post permanency services for adopted children to help assure permanency of a legal family; and to ensure permanence, stability and continuity for the children in these families through adoption or legal guardianship.
- Objectives
• The total number of disrupted adoptions will decrease.
• There will be a single point of contact for information, support, and direction.
• Information and resources will be provided to adopted children and families seeking community resources.
• Consultation and intervention will be provided to adopted children and families in crisis.
• Adopted children and families will be connected to community resources.
• Advocacy and system navigation will be provided for adopted children and families experiencing a crisis.
• Adoptive placements will be stabilized by connecting families to supportive services in the community during the period of supervision to prevent disruptions or to prevent dissolutions after finalization.
• Adopted children and families will remain intact by preventing the child from entering or re-entering the foster care system.
• Building strong collaboration with child welfare partners to strengthen the service continuum for families and children.
