Survey Data Collection Partner Outreach Services

The Vendor is required to provide a comprehensive data collection effort to better understand the health issues, strengths, and challenges of a community.
- Health conducts in collaboration with community organizations, municipalities, and healthcare partners, to develop a deeper understanding of issues related to quality of life, availability of essential services, and physical and mental health.
- Implement the approved outreach and engagement plan beginning February 2026, with the intent of increasing agency participation from the proposed target population(s).
- Distribute agency-provided or approved materials to the proposed target population(s) as outlined in the plan.
- Utilize engagement techniques that are culturally and linguistically appropriate for target population(s).
- Adhere to the agency -provided data collection protocols distributed during the initial training.
- Paper surveys must be submitted to agency within 2 weeks of completion to ensure data integrity and confidentiality.
