The Vendor is required to provide primary care and medical director services to the residents at the home in location.
- provide services on site, and establish policies for medical practice, coordinate medical care and assure the appropriateness of medical care to all residents within the facility
- It provides a safe, supportive environment with a mission to deliver high-quality, resident-centered care with dignity and respect. The home offers comprehensive medical, nursing, and rehabilitative services alongside therapeutic recreation programs to all residents, including a life enhanced dementia unit.
- On site primary care and medical director services is being solicited to provide services on-site to the agency.
- A felony for child abuse or neglect, spousal abuse, any crime against children or adults, including but not limited to: violent or sexually-related crime against a child or adult, or a crime which may indicate a person might be reasonably expected to pose a threat to an adult; and felony for physical assault, battery, or drug-related offense committed in the past five years.
- The vendor and staff must comply with the facility policies. Policies must be reviewed and signed annually.
- The Medical provider will be responsible for their required license fees, insurance, taxes and any other expenses incurred in order to provide services at the agency location.
- Provide medical services in accordance with all applicable federal, state, and local laws, rules, and regulations, and third-party reimbursement requirements (public or private) and agrees to maintain all current licensures needed to provide medical services in the State.
- Perform all duties and render such services required to maintain the health and physical welfare of all residents at the Home.
- Ensure that all residents are examined once a month if their physical and mental condition warrants. Such record of examination shall include the ordering of labs and radiology (if necessary).
- Collaborate with Pharmacy and Director of Resident Care Services
- Collaborate with staff development regarding in service training programs.
- Attend monthly medical meetings in person.
- Participates in establishing policies and procedures along with guidelines to ensure adequate, comprehensive services and reviews. Serve, in person, on Quality Assurance Committee (quarterly) and provide consultation for infection control.
- Attend anti-microbial stewardship on site, quarterly.
- Advanced gero-pharmacology knowledge is preferred.
