The vendor is required to provide comprehensive pre-employment screening services on an as-needed basis.
- Services may include pre-employment medical examinations, drug and alcohol testing, psychological evaluations, and other related assessments necessary to determine candidate fitness and suitability for employment with the city.
- The city intends to procure professional screening services on an as-needed basis.
- Pre-employment medical examinations and evaluations for prospective employees. the objectives of the city’s pre-employment exam process are to ensure that all candidates for city employment are:
• Medically capable of performing job duties
• Not placed in positions that could aggravate existing medical conditions or create hazards to themselves or others
• Placed in roles that permit maximum accommodation of individuals with disabilities physiologic measures required of all applicants will include, but are not limited to: blood pressure, pulse, respiration, temperature, height, weight, visual acuity, color vision, and urinalysis.
- Occupational and environmental health exams and evaluations, such as respiratory or hearing screenings.
- Initiate and update and maintain medical/health records on all employees and applicants for whom the contractor provides services. health records shall include, but are not limited to, the following:
• Health questionnaire
• Results of all tests
• Proofs of immunity
• Examinations
• Physician consultations
• Progress notes with rationale for any additional tests or reports beyond of a usual examination
- Maintain and store all medical records for the term of the contract, at no additional cost, in a secure and confidential manner.
