The Vendor is required to provide to conduct extensive pre-employment background investigations for sworn fire personnel, for the city.
- Responsible for developing, conducting, and reporting on background investigations for approximately 50 potential candidates each year.
- Services include:
1. Collect all waivers and necessary forms:
• Advisory Waiver
• Applicant Wavier
• Background Consent
• Pre-Employment Background form
• Personal History Statement (PHS)
• Substance Abuse Statement
• Medical / Physiological Clearances
2. Collect Personal / Legal Records:
- Fingerprint response via Live Scan
- Driving Record
• Copy of Driver’s License
• Proof of Insurance
• Vehicle Registration
• Accident reports
- Law Enforcement Checks:
• County
• Court Records
• NCIC
• Interpol
• All other agencies (include every county where applicant has resided, attended school or worked.)
- Citizenship Documents:
• Birth Certificate
• Immigration Papers (If Applicable)
• Naturalization Papers (If Applicable)
- Military DD214/ Selective Service
- Marriage / Dissolution of Marriage Certificates
- EDD Inquiry
- Collect and Report Education Verification
• High School Transcripts
• GED/ CA High School Proficiency Exam (If applicable)
• High School Diploma
• College Transcript(s)
• College Degree
- Training Certificates
- Letter of Recommendation.
- Contract Period/Term: 5 years
