The Vendor is required to provide for pre-employment screening services.
- Reputable and properly licensed physicians or medical facilities.
- To provide certain medical services in connection with employment matters for the City
- Services available to meet the needs of the City
- List and describe affiliations with hospitals or hospital emergency rooms.
- Staff size and credentials of personnel who will be providing pre-employment screening services to conditional City employees.
- Specify who will be assigned to the City and who will provide backup services in the event the original designee(s) is unavailable.
- Provide resumes of medical staff.
- The directing physician should be board certified in Occupational Medicine.
- City management shall designate and specifically approve all physicals including specific components of physicals at the time of service.
- Vision Screening
- Provide sample copies of any forms to be used in the pre-employment screening of conditional City employees.
- Drug screening, including confirmation testing, if necessary, shall be performed for all new hire positions.
- The physician shall follow the drug test confirmation procedure established by law or regulation in the event a positive drug test is received.
- Contract Period/Term: 1 year
