The Vendor is required to provide comprehensive occupational health and screening services for employees participating in the department.
- These examinations include medical questionnaires, physical exam, spirometry (when needed), complete blood count, chemistry lipid panel, job hazard assessment, and the evaluation of screening results by a physician to assess employee fitness for respirator use in compliance with the acts.
- The clinic should provide the service request form with employees’ personal information, services that need to be performed, and locations of clinics throughout state where employees will select for their appointment.
- Respirator Medical Evaluation
• Shall be conducted online or in-person. If any follow-up is required with online submission telehealth option shall be used as long as it does not compromise the integrity, performance, or validity of required testing or medical evaluations
- The employee’s height and weight using a balance beam scale and stadiometer will be determined and recorded. Height and weight parameters will be used to calculate various other physiological and anatomic values. Height will be taken without shoes. Weight will be taken with indoor clothing and without shoes.
- Blood pressures will be measured using sphygmomanometers or automated blood pressure monitor. The employee will be in a sitting position during the reading. If blood pressure results exceed either systolic (140) or diastolic (90) reference range, the test will be repeated after the employee has been at rest for 10 minutes. If the employee has been diagnosed as hypertensive, all medications that the employee is using to control the hypertension will be recorded.
- A radial pulse will be measured manually or by automated pulse monitor. The pulse rate and rhythm will be recorded.
- It will be recorded if the employee is overweight. Any posture defects, perceptible limp, tremor, or other defects that might be caused by alcoholism, thyroid intoxication, or other illnesses will be noted.
- Any evidence of disease or injury involving the eyes, ears, nose, sinuses, and throat, including any that could interfere with breathing or use of mask respirators will be recorded. Ears: Any evidence of mastoid or middle ear disease, tympanic membrane perforation or scarring, discharge, symptoms of vertigo, or Meniere’s syndrome will be recorded. Teeth: Dentition must be adequate to support the normal facial structures. Note any evidence of deformities of the teeth, loose teeth, or prosthetic devices that could interfere with breathing.
- Stethoscopic examination is required. Note murmurs and arrhythmias, and any past or present history of cardiovascular disease of a variety known to be accompanied by syncope, dyspnea, collapse, enlarged heart, or congestive heart failure. Perfusion of the extremities must be included in the cardiac assessment. List any diagnosis such as angina, arrhythmias, status post myocardial infarction, ventricular septal defect, or other factors, and medications the employee uses to control symptoms related to the disorder.
- Color vision: Ability to distinguish colors assessed using Ishihara plates. If unable to distinguish Ishihara, must be able to distinguish red, green, amber, color felts.
- It is the responsibility of the Medical Surveillance Program director to determine if treadmill testing is indicated for a particular employee.
