The Vendor is required to provide strategic plan and quality improvement consultant services for include:
• A quality improvement plan timeline.
• To be finalized one week after notice of award;
• Conduct a needs assessment by reviewing existing policies, plans, and performance management practices.
• Assess current QI capacity, infrastructure, and culture within department.
• Develop a written quality improvement plan
• QI governance structure (e.g., QI council or committee), roles and responsibilities of staff and leadership;
• Standardized QI methodology
• Criteria for selecting, prioritizing, and documenting QI projects;
• Performance measurement and reporting processes;
• Communication strategies for quality improvement (QI) activities, and;
• Integration with the performance management system
• Ensure the QI plan meets department requirements and aligns with department strategic and workforce development plans.
• Incorporate relevant documentation and processes to support accreditation readiness.
• Provide one training to the QI advisory committee on the plan and associated processes;
• State health improvement plan (ship);
• Community health improvement plan (chip);
• Achieving health care efficiency through accountable design (ahead), and;
• The quality improvement plan.
