The vendor is required to provide pharmacy services for include:
- Regulatory
• Comply with all legislative, regulatory and licensing requirements
• Demonstrate evidence of ability to meet core services as outlined in the supplementary standards for pharmacists caring for residents of long term care homes
• Address philosophy and requirements of authority guiding documents as listed above
- Quality and safety
• Comply with authority privacy policy
• Comply with applicable accreditation standards
• Participation in quality improvement initiatives
• Participation in medication incident reviews and reporting community
• Identify and minimize diversion opportunities within medication process
• Demonstrate evidence of ability to provide medications in a safe and efficient manner
• Patient advocacy for things such as supplemental support, palliative care coverage, exceptional drug status, ensuring prescriber and resident are aware of the cost implications of prescribing choices, appropriate prescribing, de-prescribing, etc.
- Clinical collaboration
• On-site medication management process for each resident’s medications
• Provide consultation for the development of pharmacy related policies and procedures
• Participate as a member of the interdisciplinary facility team during path of life meetings and care conferences and medication reviews
• Active communication methods with the healthcare team to ensure safety and health of the residents and staff
- Service and delivery
• Demonstrate evidence of a risk management plan including information on ability to provide medications during service disruptions due to unusual circumstances (e.g. Loss of power, transportation during severe weather, etc.)
• Billing procedures that would be used – frequency, responsibility to collect, credit process for returned medications that can be re-dispensed, sample bill, costs for repackaging, any additional charges
• Demonstrate ability to comply with authority electronic requirements
• Monitor dosage system to be used, also include dosage system for prn and multi-dose medications
• Hours when pharmacy services would regularly be available and method of obtaining afterhours emergency services
• Process to provide medication education for residents, staff, and families
• Participation in annual family care conferences
• Audit process for medication room, medication system, controlled substances, night cupboard, and medication reconciliation.
• Identify process for delivery of narcotics and controlled drugs
• Identify process for managing pass medications
• Provide sample documentation or demonstration of electronic medication administration record
• Provide availability to participate in local, network and provincial committees
• Optimize medication management to improve health outcomes for residents
• Any other additional services that will be provided to the resident
• The comprehensive medication management process
• Define process for the confidential disposal and destruction of medications and confidential disposal of the packaging in the case of patient identifiers present.
• Process for transition of resident information in the context of transition of services either with a change of location of resident or a change of contracted provider.
• The medication reconciliation process at transitions of care
• Complaint management process for residents.
• Include med pouch disposal, stat box and emergency box listing.
