The vendor is required to provide to deliver case management services to participants of the medical day care services waiver (MDCSW) program.
- Case managers from the CMAs assist with accessing and maintaining Medicaid coverage while coordinating community services and support from various providers and funding sources to develop a comprehensive POS for successful community living.
- Medical day care services waiver (MDCSW)
• Provides in-person services and supports to medically, technically, and financially eligible individuals aged 16 and older.
• These individuals must be eligible for community Medicaid, meet a nursing facility level of care (loc), and choose AMDC services as a cost-effective community-based alternative to institutional care.
• MDCSW services may include, but are not limited to, nursing services performed by a licensed practical nurse under the supervision of an RN, occupational and/or physical therapy services, nutrition services, social work services, activity programs, and transportation services.
• The centers also provide critical medical services to prevent or delay institutionalization while providing critical, social, cognitive, and physical stimulation to preserve independence.
• The reimbursement for the MDCSW services is an all-inclusive per diem rate for skilled nursing services, nursing assessments, medication administration and monitoring, social work services, activity programming, assistance with activities of daily living (ADLs), daily living skills training, transportation for access to medical appointments and therapies, personal care, meals, and nutrition services provided by the adult medical day care centers (AMDC) center or AMDC subcontractor in the community.
• Contracted services can include, but are not limited to, therapy services to provide physical, occupational, and speech therapies or caterers to provide meals.
• Other Medicaid divisions or programs involved in the operation of the MDCSW include department and the office of systems, operations, and pharmacy, which manages provider enrollment and service reimbursement through the Medicaid management information system (MMIS).
- MDCSW application process and receiving case management
• Individuals who reside in the community and have community medical assistance can apply for MDCSW services at any time, and no additional application is required to receive MDCSW or case management services.
• After confirming the nursing facility loc, agency will provide applicants with an information packet, including brochures from all eligible case management agencies (CMAs).
- Ongoing case management
• Once an individual is enrolled in MDCSW, the case manager will maintain contact with the participant to ensure their needs are met with the services and supports outlined in the POS and identify any unmet needs.
• The case manager must meet with the participant at least once every 180 days.
• Unless otherwise specified by the department, the case manager will meet with the participant in person at the AMDC center or in a place and time preferred by the participant.
• The CMA will also review all available resources to ensure the provided services are consistent with the POS.
