RFP Description

The Vendor is required to provide medical and occupational health services to employees, retirees, and covered dependents who are enrolled in the town health plan.
- Primary care
• Preventive care and biometric screening
• Health risk assessments
• Lab work and vaccinations
• Annual physicals
• Chronic condition management
• Acute healthcare, symptom treatment and management
• Medical care for urgent conditions
• Wellness program management
• Disease management
• Audiology testing
• Minor diagnostic services
• Follow up care for abnormal screening results
• Dispensary medication services
• Collaborate with the town to promote initiatives including health education promotions and strategies, such as but not limited to, preventive health screenings, vaccinations, weight management programs, prevention awareness activities and events, and tobacco cessation services.
- Occupational health
• First treatment of minor work-related injuries; must be able to coordinate with the town’s workers compensation TPA.
• Routine pre-employment physicals, including history and physical examination
• Police and fire department pre-employment and annual physicals, including history and physical examination
• Department urine drug screening
• Non-department urine drug screening
• Breath alcohol testing
• Department physical
• Audiometric testing
• Respiratory testing
• Fitness for duty exam/return to work exams
• Post offer physical & drug screen
• Record keeping ability to track and report on workers compensation claims
• Additional services as needed
- Facilities and staffing
• Operate one clinic in the town, in a location that is convenient for town plan participants.
• The bidder is to secure necessary clinic facility and clinic provider licenses if applicable.
• Manage and equip the clinic facility with necessary medical equipment & technology.
• Facility is to include limited medication dispensing, laboratory services, and radiology services, if financially feasible.
• Hours of operation are to be flexible to meet the needs of town plan participants.
• Clinic staff members are to be contracted or regular (full time and regular part time) employees of the bidder.
• The town reserves the right to review and approve all clinic staff during the term of the contract.
• The clinic is to be staffed with either a certified physician, physician’s assistant, or a nurse practitioner.
• The clinic is to have a system in place to follow best practices and evidence-based medicine guidelines.
• Clinic staff members are to be able to identify and act on gaps in medical and medication care.
• Clinic staff members are to contact town plan participants following medical and occupational encounters to discuss compliance and conditions.
