The vendor is required to provide services of a contractor to deliver virtual professional development healthcare trainings targeted to community health workers (CHWs).
- The instructional and supporting materials will be provided by office of population health improvement (OPHI) for the trainings in the following five (5) healthcare areas: behavioral health, chronic disease: asthma, infectious disease, maternal and child health, and older adults and aging.
- The successful contractor will be responsible for:
• Collaboration with OPHI on outreach to potential trainees;
• Registration for all trainings;
• Delivery of three (3) virtual trainings on each healthcare area at dates and times agreed upon by the contractor and OPHI;
• Conducting an assessment of learning by participants in each training session;
• Overall evaluation of the trainings; and
• The completion of a final report.
- Statement of work:
• Demonstration of understanding of the CHW workforce.
• Demonstration of understanding of the deliverables in this RFP and their alignment with CHW program goals, to include the responsibilities and tasks and the deliverable requirements
• Demonstration of understanding of the work of all elements necessary to implement the responsibilities and tasks of this RFP
- Work plan:
• APPROACH to the scope of work and deliverables.
• Approach to overall and task-specific management within stated timeframes.
- Department obligations:
• Provide the five (5) specialty healthcare module training materials;
• Collaborate in the outreach efforts to potential registrants;
• Provide guidance concerning contract activities, as needed;
• Evaluate and accept the summary report of participant training enrollment, completion, and knowledge gained; and
• Approve and submit invoices for payment in a timely manner.
