The Vendor is required to provide an integrated community resource database and referral delivery system: most of our patient population (75%) has Medicaid or Medicare insurance and a fixed income.
- Requirements:
1. Platform
• The platform must be accessible via the electronic health record (EHR) and via web platform for staff without EHR access, as needed.
• A web-based version of the platform that can be provided directly to customer patients is highly desirable.
• The platform should be persistently available except for scheduled downtimes for maintenance.
• Technical support should be available 24 hours a day, 7 days per week with live support options.
• Be able to list and manage internal programs
• The ability to auto-generate lists of appropriate resources related to completed social determinants of health (SDOH) screenings in the EHR.
2. Technical specifications:
• Fast healthcare interoperability resources (FHIR) integration with the EHR is required.
• EHR integration must include an EHR workflow application, with the ability to access the platform and receive a social care summary write back through our existing EHR system instance, and the ability to securely retrieve patient contact information from the chart and the ability to store referrals in the EHR.
• May include implementation of administrative, physical, and technical safeguards to protect the confidentiality, integrity, and availability of patients' electronic protected health information (EPHI); may include measures like access controls, data encryption, risk assessments, and incident response plans.
• Supply methods to transmit community resource information clearly, securely, and quickly via customer’s preferred methods of communication, which must include SMS text message, email, epic my chart message, and print
• Resources must be available in the database of “community resources” where customer engagement with the referral can be recorded by the receiving resource and visible to the end staff and in the provided reporting tool or platform as well.
