Cancer and Chronic Disease Data Management System

The Vendor is required to provide services for enhancement and maintenance of the cancer and chronic disease data management system.
- The center of a comprehensive network of public health services, establishing public health priorities and evaluating the effectiveness of health initiatives
- This will align federal grant requirements, internal state operations with external clinical operations with SFL clients, comprehensive cancer program providers and health systems which will allow for ease of data collection, production of federal/state reports and establish a more robust centralized hub to systematically support decision making.
- Develop queries for often viewed reports based on selected date range.
- This database works simultaneously with the Smart Sheets data collection tool to collect specific points of data relating to all BCPC and CDB programs.
- the Maintenance and Operation of the Screening for Life (SFL) system, which will be renamed to the Cancer and Chronic Disease Data Management System (CCDDMS) to better align with the goals of the programs.
- This front-end data collection system is used to support Department response to statewide strategies aimed at mitigating the impact of chronic disease risk factors affiliated with cancer, hypertension, diabetes asthma and other chronic conditions.
- The Project Director will serve as primary coordinator to ensure that Joint Application Design (JAD) sessions take place with the appropriate subject matter experts (SME), that project documents and deliverables are thoroughly reviewed, and that approval takes place within agreed upon timeframes.
- The Project Director will coordinate with other divisions and State agencies for their input as needed. These staff will serve primarily as subject matter experts on relevant Division applications and related systems and will participate in meetings and deliverable review as necessary.
