The Vendor is required to provide behavioral health risk monitoring and coordination software system should enhance the department’s ability to identify emerging risks, facilitate appropriate interventions, and document actions taken across the continuum of care.
- Requirement:
1. Structured rounding and monitoring tools
• Configurable workflows to support monitoring activities, including outreach, wellness checks, and provider follow-ups
• Documentation of client contacts and observations
• Assignment and tracking of follow-up actions across responsible entities
• Ability to facilitate communication and information sharing among authorized members of a treatment team
• Ability to identify outstanding follow-up activities and support timely communication regarding changes in risk or clinical status
2. Event and incident reporting
• Centralized system for reporting and tracking clinical, behavioral, safety, and compliance-related incidents
• Ability to capture events and relevant case information from multiple provider agencies and authorized treatment team members
• Severity classification and escalation processes for high-risk situations
• Comprehensive audit trail documenting reporting, review, follow-up, and resolution activities
• Ability to aggregate and export incident data for reporting, trend analysis, quality improvement, and program evaluation.
