The Vendor is required to provide are as needed, statewide biomedical destruction services (by way of incineration and landfill according to applicable local, state and federal laws) for state owned goods (such as supplies, equipment, pharmaceuticals and biomedical waste) for the department.
• Dispose of recyclable items that can be repurposed or reused such as metals, plastics, and papers at recycling centers appropriate to the items and performed in strict accordance with federal, state, and local codes and ordinances and shall comply with all applicable commission on environmental quality, agency, occupational safety and health administration (osha) to reduce the burden on landfills and environmental pollution.
• Equipment and supplies may not be salvaged or reconditioned in whole or in part for sale or redistribution.
• Responsible for destruction of liquid and solid mixture materials and solid without free liquid materials from department contracted storage locations.
• Responsible for supervision, clean up and disposal, and must have all applicable licenses, permits and insurance coverage.
• Also be responsible for safe and legal disposal of infectious waste: materials contaminated with blood or body fluids (bandages, gloves, and gowns), pathological waste: human tissues, organs, body parts and fluids and chemical and laboratory waste: reagents, disinfectants and laboratory cultures.
• Provide the following information on the service ticket: project name, name of personnel performing the work, trade category of person performing work, number of personnel, and hours worked.
• Invoice must match approved service tickets.
• Material invoices shall be attached to final invoice.
