The Vendor is required to provide for a lead family support advocate (FSA) to provide support with the statewide outreach center (SOC) intervention care coordinator by providing oversight on data collection of newborns who did not initially pass the hearing screening.
- Primary location of services will be remote.
- Provide family support advocate training prior to new contractors conducting any project related activities.
- Facilitate conference calls to review the project, discuss progress, setbacks, concerns, successes and recommendations for next steps as needed.
- Support the development and revision of procedures related to the project.
- Strategies will include direct education and outreach from the parent perspective.
- Attend strategy planning, orientation and training meetings.
- Contract Period/Term: 1 year
