Community Bases Care Project Reporting Services

The Vendor is required to provide the services required to assist with the planning, implementation, and completion of the Community-Based Care (CBC) Project (project). 
- These services will include simulation modeling using department data, analyses, and interpretations of modeling results, and the expertise, knowledge, and experience to successfully complete the project’s objectives.
- Reporting will support outcome evaluation of CBC testing the theory of change and logical relationships between inputs and outcomes.
- Reports will pertain to the established project outcomes: permanency, placement stability, placement reentry, and placement proximity.
- Contract Period/Term: 1 year
