The Vendor is required to provide to supplement and assist with the implementation of a comprehensive community outreach strategy that improves awareness of the district and its system entities, agency, as well as the programs and services offered by the entities.
- Must have and demonstrate the following:
• Experience in grassroots, community-based outreach
• Culturally and linguistically appropriate engagement strategies
• Relationships and experience with historically underserved or hard-to-reach populations, specifically in person and in County and/or the neighboring counties, including but not limited to the following communities:
o Non-English speakers, especially Spanish, Arabic, and Asian language speaking groups, such as Vietnamese, Korean, Burmese, and Farsi
o Black/African American
o Latino
o Asian
o People experiencing homelessness
o Justice-involved individuals and those coming out of incarceration
o People who use drugs
• A Community Health Worker model with staff representative of the communities they serve
- Grassroots Outreach
• Developing and implementing strategies to reach residents in nontraditional settings (e.g. encampments, food pantries, faith-based sites, reentry programs).
• Assisting, as needed, in gathering information from the target communities, evaluating the community/communities’ interactions and engagements with the healthcare district’s entities, and/or providing feedback on the services and engagement opportunities provided by the healthcare district.
• Disseminating culturally and linguistically appropriate outreach materials in multiple languages (including but not limited to Spanish, Vietnamese, Burmese, Farsi, Korean, and Arabic). These materials will be developed by the District.
• Applying trauma-informed, culturally responsive outreach practices that promote trust and reduce stigma around seeking health care.
• Promoting participation in the healthcare district’s programs, services, and public engagement opportunities.
- Application Assistance
• Provide one-on-one and/or small-group support to help residents translate and complete applications for the healthcare district’s services and programs, such as the Medical Access Program (MAP).
• Offer mobile and site-based assistance at community hubs, libraries, shelters, reentry facilities, and other community spaces as needed.
• Maintain confidentiality and accuracy when handling applicant information.
• Have staff complete training on eligibility, to be provided by the District.
- Community Resource Referrals
• Providing guided referrals and warm handoffs to social and health resources, including food, housing, behavioral health, and reentry support.
• Maintaining records of referrals and tracking follow-up outcomes where possible.
• Collaborating with partner agencies to identify gaps and improve referral pathways.
