The Vendor is required to provide co-operating the school-based health centers (SBHC), ensuring the delivery of comprehensive medical, mental health, dental, vision, and case management services.
- Medical Services
• Routine physical exams, including sports physicals.
• Well-child visits.
• Immunizations, in compliance with state and local requirements and recommendations.
• Diagnosis and treatment of acute illnesses and minor injuries.
• Chronic disease management (e.g., asthma, diabetes).
• Reproductive health services as permitted by state law.
• Referrals to specialized physical health services as needed.
- Behavioral/Mental Health Services
• Individual, group, and family counseling/therapeutic sessions.
• Crisis intervention and suicide prevention services.
• Substance use prevention and intervention programs.
• Referrals to specialized mental health services as needed.
- Dental Services
• Preventive care, including cleanings, fluoride treatments, and sealants.
• Diagnosis and treatment of common dental issues, including restorative care.
- Vision Services
• Vision screenings and comprehensive eye exams.
• Provision of corrective eyewear (glasses or contact lenses) when needed.
- Case Management Services
• Facilitating access to comprehensive health services available at the SBHC
• Connecting students and families to external resources such as housing assistance, food security programs, and educational support services
• Benefits counseling to connect families to necessary, available services
- Health Education
• Provide health education on topics like nutrition, mental health, and disease prevention.
• Facilitate workshops and group sessions on health-related topics.
• Promote healthy behaviors through outreach and awareness campaigns.
- Ability to provide medical and mental health services to qualifying department staff.
- Provide on-site or mobile clinic services to schools within the geographic cluster.
