RFP Description

The vendor is required to provide comprehensive occupational medical program services for fire medical examinations must include:
a. Comprehensive medical history – NFPA 1582 7.5
• All necessary forms will be provided including osha respirator questionnaire
b. Physical examination – NFPA 1582 7.6
• Vitals (temperature, pulse, respiratory rate, and blood pressure)
• Head, eyes, ears, nose, throat (HEENT)
• Neck
• Heart
• Lungs
• Breast
• Gastrointestinal with digital rectal exam as clinically indicated
• Hernia
• Lymph nodes
• Neurological
• Musculoskeletal
• Skin, thyroid, and testicular cancers
• Vision
c. Blood tests – NFPA 1582 7.7.1 and additional
• CBC with differential
• RBC indices and morphology, and platelet count
• Electrolytes (NA, k, cl, hco3, or co2)
• Renal function (bun, creatine)
• Glucose
• Liver function tests (alt, AST, direct and indirect bilirubin, alkaline phosphatase)
• Total cholesterol, HDL, LDL, clinically useful lipid ratios, and triglycerides
• PSA (starting at age 40)
d. Urine laboratory tests – NFPA 1582 7.7.2
• Dipstick analysis for NFPA, ketones, leukocyte esterase, protein, blood, and bilirubin
• Microscopic analysis for RBC, WBC, casts, and crystals if indicated by results of dipstick analysis
• Analysis of occupational chemical exposure if indicated
e. Hearing test – NFPA 1582 7.7.3
f. Spirometry – NFPA 1582 7.7.4
g. Chest radio graphs – NFPA 1582 7.7.5
• Chest x-rays shall include an initial baseline and will be repeated as medically indicated
h. Electrocardiograms – NFPA 1582 7.7.6
• A resting 12 lead ECG shall be performed as part of the baseline medical evaluation and shall be performed annually after age 40
i. Mammography – NFPA 1582 7.7.8
• Mammography shall be performed on each female member biennially for those over the age of 40 and annually for those over the age 50 or as clinically indicated.
j. colon cancer screening – NFPA 1582 7.7.13
• Risks and benefits of screening shall be discussed with all members above the age of 40 or earlier is clinically indicated. (Stool based or colonoscopy)
k. Prostate cancer – NFPA 1582 7.7.14
• Risks and benefits of cancer screening, including PSA, shall be discussed beginning at age 50 and annually thereafter.
• Firefighters at higher risk for prostate cancer, the physician shall discuss the risks and benefits of prostate cancer screening, PSA testing will start beginning at age 40 and annually thereafter.
l. Hormone imbalance awareness consultation – NFPA 1582 7.7.25
• During the annual physical, the fire department physician shall inform the member of the heightened risks of abnormal hormone levels, high or low, that can have adverse effects on a member’s health.
m. Behavioral health screening – NFPA 1582 7.7.26
• Depression (phq-9)
• Anxiety (gad-7)
• Alcohol use (audit)
• Post-traumatic stress (pcl-5)
• Impaired sleep (isi-7)
• Positive psychological factors
• Meeting with a mental health professional to discuss results
n. Immunizations and infectious disease screening – NFPA 1582 7.7.9
o. Heavy metals evaluation – NFPA 1582 7.7.12
• New employees will collect a baseline test.
• After that employee will be tested in correlation with an exposure.
