The vendor is required to provide families and county case workers with medical support services and medical guidance on how to move forward while working with identified families.
- This approach will increase access to health care and community-based resources that support improved health outcomes for children and families through home visits conducted by the contractor.
- Contractor shall conduct a thorough on-site medical assessment of the referred child(ren), including weight, growth and development, health review, confirmation of immunizations, etc. and obtain necessary medical consents.
- Contractor shall enquire about, and confirm, any ongoing care and refer the family, as needed, to ongoing medical care resources.
- Provide consultations for families with children who have medically complex conditions, providing families with access to medical training on issues that apply to their child’s condition.
- These services include but are not limited to participating in child welfare intake investigations when a new case is being established; determining if child abuse / neglect is present; monitoring ongoing county cases regarding healthcare issues such as child nutrition; observation of proper child physiological and mental development; assessment of child(ren)s environmental, psychological, and mental health; evaluating health related behavior; and parenting style effectiveness.

1. Outcomes
• Provide families with knowledge of their child’s condition and access to care for the long-term medical needs of their children.
• Provide medical intervention at the appropriate time to assist the family with obtaining quality and suitable care and understanding the ongoing medical needs of their child.
2. Reporting:
• The number of child abuse and neglect referrals received by the contractor.
• The number of referrals that receive at least one successful home visit where the contractor has successfully engaged with the family.
• The number of home visits and medical assessments conducted by the contractor.
• The number of families that had current/ongoing care in place with a medical provider at the time of the assessment.
• The number of families that had no current/ongoing care in place with a medical provider at the time of the assessment.
• The number of families connected with ongoing medical care after the home visit and assessment.
• A list of trainings provided to county staff and the date of the training.
