The vendor is required to provide oversees and coordinates statewide chronic disease prevention and health promotion programs to reduce the risks of chronic disease through policies, systems, and environmental changes; develops comprehensive approaches to support healthy lifestyles that are tobacco-free, based on good nutrition and regular physical activity, and improve the quality and years of life for those living with chronic diseases, including asthma, cancer, diabetes, heart disease, and stroke.
- To provide a coordinated approach to promote physical activity, healthy nutrition, and tobacco-free lifestyles and chronic disease management and control at the state and community level.
- Project assignments shall be in support of state general, federal, special, and trust-funded projects designed by the state for which recurring public health programs and coalition management and coordination services that strengthen, support, and mobilize communities are required.
- Provide complete the work within the service period of the contract.
- Provide the state with a task-by-task project work plan schedule with a timeline for key deliverables or services, which includes the project schedule, milestones, resource requirements, and all relevant benchmarks.
- The vendor provide awarded this project must have extensive experience in public health coalition building around sectors related to healthy eating and active living (heal), access to locally grown food, active transportation and multi-modal networks, and tobacco prevention and cessation; and public health strategies including strategic partnership development and community engagement, awareness and education, promotion of health equity, and public health policies.
- The vendor will need to provide documentation of experience.
- Provide the necessary project staff during the term of the contract as requested by the state; the contractor’s staff or subcontractors (if any) shall possess sufficient skill and experience and be capable of properly performing their tasks as specified by the state.
- Provide the following services for all projects on all specified islands identified by the state during the contract period in a timely manner as directed by the state; all expenses made by the contractor shall be approved by the state prior to purchasing.
- Participate in an initial orientation and discovery meeting to coordinate, collaborate, and plan, in consultation with the state, to identify the service tasks needed to organize all identified projects on all specified islands for the state; this service shall be approved by the state and may include subsequent follow-up meeting(s) between the contractor and the state to identify the tasks needed for each identified project.
- Contract Period/Term: 1 year
